OBJECTIVE: This systematic review/meta-analysis investigated the influence of NaOCl on cyclic fatigue resistance of endodontic NiTi instruments. MATERIALS AND METHODS: A systematic search until July 2022 in PubMed/MEDLINE, Embase, Scopus, Web of Science, SciELO, Cochrane Library, and grey literature was conducted. According to the PECOS strategy, only in vitro studies evaluating the effects of NaOCl on the cyclic fatigue resistance of NiTi instruments were eligible. Cyclic fatigue resistance was the primary outcome. A modified Joanna Briggs Institute's Checklist was used for risk of bias assessment. RESULTS: Of the 2,445 records screened, 37 studies were included. Most studies used simulated canals made of stainless-steel block with severe to moderate curvatures. NaOCl concentration varied from 1-6%, mainly at 37 °C. Regarding fatigue resistance, 23 studies using 1.2% to 6% NaOCl showed a reduction in the resistance compared to the control groups, especially when pre-heated. Four meta-analyses were performed according to the tested NiTi systems. The meta-analyses indicated that the PTU F2 files had higher reduction of fatigue resistance after exposure to 5.25% NaOCl; no differences between NaOCl and no immersion were observed for Reciproc R25, WaveOne 25.08, and WaveOne Gold Primary files. Included studies had low risk of bias. CONCLUSION: NaOCl appears to reduce cyclic fatigue resistance of certain NiTi files, especially when they are pre-heated, particularly in conventional NiTi files compared to some heat-treated instruments. It is possible that the temperature of the solution may have a greater influence on resistance than NaOCl itself. Important to note that an overall tendency toward no significant influence was observed among various systems. CLINICAL RELEVANCE: Precautions are necessary when a pre-heated high-concentration NaOCl is used to enhance its properties during root canal preparation, mainly using conventional wire.

Foraminal enlargement has been recommended to optimize the disinfection of infected root canals, although some authors still claim that the foramen should be kept in its original shape and position. This study aimed to evaluate morphological alterations of apical foramen after foraminal enlargement through a systematic review. An electronic search was conducted until April 2022. Ex vivo studies evaluating influence of foraminal enlargement in the morphologic changes of apical foramen were included. Studies without a control group or available full text were excluded. Foraminal deformation and area increase were considered as primary outcomes. Risk-of-bias assessment was performed according to a modified Joanna Briggs Institute's Checklist. From 702 studies retrieved, five were eligible. Most studies used single-rooted teeth, and rotary systems for instrumentation ranging from ­ 2 mm to + 1 mm to the apex. All studies found increased major foramen deformation after foraminal enlargement. Among four studies that evaluated foraminal area, all found increased area after foraminal enlargement. Insufficient data for touched/untouched walls by instruments and dentinal microcrack formation was observed. A low risk of bias was found. Foraminal enlargement during root canal preparation seems to increase deformation and major apical foramen area. Future investigations with standardized methodologies are encouraged (AU)


A ampliação foraminal tem sido recomendada para otimizar a desinfecção de canais radiculares infectados, embora alguns autores ainda afirmem que o forame deve ser mantido em sua forma e posição originais. Este estudo teve como objetivo avaliar alterações morfológicas do forame apical após ampliação foraminal por meio de uma revisão sistemática. Uma busca eletrônica foi realizada até abril de 2022. Foram incluídos estudos ex vivo que avaliaram a influência da ampliação foraminal nas alterações morfológicas do forame apical. Foram excluídos estudos sem grupo controle ou com texto completo indisponível. A deformação foraminal e o aumento da área foram considerados desfechos primários. A avaliação do risco de viés foi realizada de acordo com uma lista de verificação modificada do Instituto Joanna Briggs. Dos 702 registros recuperados, cinco foram elegíveis. A maioria dos estudos utilizou dentes unirradiculares e sistemas rotatórios para instrumentação, com comprimento de trabalho variando de ­ 2 mm a + 1 mm até o ápice. Todos os estudos encontraram aumento da deformação do forame maior após ampliação foraminal. Dos quatro estudos que avaliaram a área foraminal, todos encontraram aumento de área após alargamento foraminal. Foram observados dados insuficientes para paredes tocadas/intocadas pelos instrumentos e formação de microfissuras dentinárias. Um baixo risco de viés foi encontrado. A ampliação foraminal durante o preparo do canal radicular parece aumentar a deformação e a área do forame apical. Futuras investigações com metodologias padronizadas são incentivadas (AU)

Objectives: This systematic review (register-osf.io/wg7ba) compared the efficacy and safety of rotary and reciprocating kinematics in the removal of filling material from curved root canals. Materials and Methods: Only in vitro studies evaluating both kinematics during retreatment were included. A systematic search (PubMed/MEDLINE, Scopus, and other databases, until January 2021), data extraction, and risk of bias analysis (Joanna Briggs Institute checklist) were performed. Efficacy in filling removal was the primary outcome. Results: The search resulted in 2,795 studies, of which 15 were included. Efficacy was measured in terms of the remaining filling material and the time required for this. Nine studies evaluated filling material removal, of which 7 found no significant differences between rotary and reciprocating kinematics. Regarding the time for filling removal, 5 studies showed no difference between both kinematics, 2 studies showed faster results with rotary systems, and other 2 showed the opposite. No significant differences were found in apical transportation, centering ability, instrument failure, dentin removed and extruded debris. A low risk of bias was observed. Conclusions: This review suggests that the choice of rotary or reciprocating kinematics does not influence the efficacy of filling removal from curved root canals. Further studies are needed to compare the kinematics safety in curved root canals.

OBJECTIVES: This systematic review and meta-analysis compared the effects of immediate and delayed post space preparation on apical sealing. MATERIALS AND METHOD: Two independent authors conducted a systematic search (PubMed/Medline, Cochrane Library, and other databases, until February 2020) and a risk of bias evaluation. Only in vitro studies that compared the effects of immediate and delayed post space preparations on apical filling and adhesion of the post were eligible. RESULT: Of the 742 articles retrieved, 32 were included. Most of the studies used single-rooted human teeth and rotary files for root canal preparation, a single-cone technique for the filling protocol, and rotary instruments for post space preparation. Various delayed preparation times were evaluated: 8-72 h, 5-30 days (mainly 7 days), and 4 months. In nine studies, the delayed groups showed more apical leakage, while four studies reported more leakage in the immediate groups; ten studies found no significant difference. One study found more bacterial penetration in the delayed group, whereas three studies showed no significant difference. One study reported more voids in the delayed group, while another found no such difference. Three studies showed better post-bond strength in the delayed group, one in the immediate group, whereas three found no significant difference in post-bond strength. Three studies employing varied sealers for root canal fillings were considered for meta-analysis. Two subgroup analyses were also performed (one concerning the use of resin-based sealers (AH Plus), another for zinc oxide-eugenol-based sealers). The results of the meta-analysis showed that a delayed post space preparation led to a significantly higher apical leakage than an immediate preparation (mean difference = 0.41 mm, confidence interval = 0.24-0.59, p < 0.001). CONCLUSION: Delayed post space preparation seems to negatively influence apical sealing; however, further studies are needed to determine the influence of the timing of post space preparation on the other parameters. CLINICAL RELEVANCE: Immediate post space preparation may be the safest clinical choice to prevent apical leakage.

This study assessed the cutting properties of two Nickel Titanium file systems with different designs and manufacturing materials. ProTaper Next X1 and X2 (PTN; Dentsply Sirona) and ProTaper Universal S2 and F1 (PTU; Dentsply Sirona) instruments were employed. The cutting parameters, that is torque and apical forces, were determined using a specially designed bench-testing machine. Dimensional and geometric parameters were considered to evaluate the results. The average maximum torque values showed statistically significant differences (P < 0.05) among all instruments. Apical force for PTN X2 and PTU F1 instruments initially decreased and then experienced a rapid increase in the last 2 mm of the canal. For PTN X1 and PTU S2, apical force increased during the entire test. When compared to PTU files, PTN required higher torque and apical forces during the shaping procedure. The results lead us to conclude that PTN instruments demonstrated a lower cutting efficiency than PTU.

INTRODUCTION: The aim of this study was to compare 2 reciprocating nickel-titanium systems with different manufacturing characteristics and to evaluate the influence of simulated clinical use on their torsional resistance. METHODS: New 25/.08 WaveOne Primary (WO; Dentsply Sirona, York, PA) and 25/.07 WaveOne Gold Primary (Dentsply Sirona) files were used in this study. The diameter and cross-sectional area at 3 mm from the tip were measured using image analyses. The alloy microstructure was evaluated by X-ray diffraction and the phase transformation temperatures by differential scanning calorimetry analyses. Forty files were divided into 4 groups: control group 1 and control group 2, which were submitted to torsion tests until rupture, and experimental group 1 and experimental group 2, in which instruments were used to shape 2 mesial canals of an extracted mandibular molar and then submitted to torsion tests until rupture. RESULTS: WO had higher values of diameter and area at 3 mm from the tip. X-ray diffraction analysis presented R-phase and austenite for both systems. Austenite finish temperatures, determined by differential scanning calorimetry, were above room temperature. WO showed higher values of torque until rupture, and WaveOne Gold showed higher values of angular deflexion (P > .05). No differences were found between the same type of instrument in the control and experimental groups. CONCLUSIONS: The higher torsional resistance of WO can be attributed to geometric characteristics. The use in 1 tooth with curved canals is safe and does not affect the torsional resistance of the instruments analyzed.

Objective The aim of this study was to evaluate the influence of torsional preloading on the cyclic flexural fatigue resistance of thermally treated NiTi instruments. Material and Methods Ten new instruments New Hyflex CM (HF 30.06; Coltene/Whaladent Inc.), Typhoon CM (TYP 30.06; Clinician's Choice Dental Products) and Vortex Blue (VB 30.06; Dentsply Tulsa Dental) were chosen, based on geometry and specific characteristics of the manufacturing process. The new instruments of each system were tested in a bench device to determine their fatigue resistance through mean value of number of cycles to failure (Nf) (Control Group - CG). Another group of 10 new HF, TYP and VB instruments were submitted to 20 cycles of torsional straining between 0° and 180° (Experimental Group - EG) and then submitted to fatigue until rupture under the same conditions of the CG. Tested instruments were examined by scanning electron microscopy (SEM). Data were analyzed using one-way analysis of variance and post hoc Tukey's test (α=.05). Results Higher fatigue resistance was accomplished by HF instruments, followed by VB and TYP (p<0.05). During the torsional preloading, the lowest mean torque value was observed for TYP instruments (p<0.05). The torsional preload caused a significant reduction in the Nf values (p<0.05) of about 20%, 39% and 45% for instruments HF, VB and TYP, respectively. Longitudinal cracks, generated during the torsional preloading, were present in VB files, but were not observed in the CM instruments (HF and TYP). Conclusions In conclusion, the flexural fatigue resistance of thermally treated instruments is diminished after cyclic torsional loading.

Aim:This study tested a setup for in vitro experimental analysis of axial forces and torque during the preparation of artificial canals using nickel-titanium reciprocating endodontic files.Methods: The cutting efficiency of Reciproc (RC) and WaveOne (WO) reciprocating size 25/.08 instruments (n = 10) was evaluated, taking into account their dimensional and geometrical features. Measurements of the diameter at each millimeter from the tip, pitch length, helical angle, and cross-sectional design and area were assessed. Cutting efficiency tests were carried out on a specific bench device by measuring the torque and axial force required during artificial canal shaping. Statistical analysis was performed using one-way ANOVA (α = 0.05). Results:The WO samples showed larger A3 mean values than did the RC instruments (p < 0.0001), despite having equal diameters at 3mm from the tip (D3) (p = 0.521). The mean values of pitch length were higher for RC than for WO instruments (p< 0.0001), with consequently smaller helical angles (p < 0.0001). For the cutting efficiency tests, the required torque was lower for the RC group when compared to the WO group, but it was significant only in the first stage of insertion in the artificial canals (p = 0.008). Regarding the apical force, the RC instruments reached higher values when compared to the WO instruments (p = 0.04) in the second stage of cutting action. Conclusion: Reciproc instruments demonstrated statistically higher cutting efficiency when compared to WaveOne instruments.


Objetivo: Este estudo testa uma configuração para análise experimental in vitro de forças axiais e torque durante o preparo de canais artificiais usando instrumentos endodônticos reciprocantes de níquel-titânio. Métodos: Foi avaliada a eficiência de corte dos instrumentos reciprocantes tamanho 25 / 0,08 (n = 10) Reciproc (RC) e WaveOne (WO), levando em consideração suas características dimensionais e geométricas. Medidas do diâmetro a cada milímetro a partir da ponta, comprimento de pitch, ângulo helicoidal e desenho da área transversal e área foram avaliados. Testes de eficiência de corte foram realizados em um dispositivo de bancada específico, medindo-se o torque e a força axial exigidos durante a modelagem de canais artificiais. A análise estatística foi feita com ANOVA one-way (α = 0,05). Resultados: As amostras de WO mostraram valores médios A3 maiores do que os instrumentos RC (p <0,0001), apesar de terem diâmetros iguais a 3 mm da ponta (D3) (p = 0,521). Os valores médios do comprimento do pitch foram maiores para o RC do que para os instrumentos do WO (p < 0,0001), com consequentemente menores ângulos helicoidais (p < 0,0001). Para os testes de eficiência de corte, o torque necessário foi menor para o grupo RC em comparação com o grupo WO, mas significativo apenas no primeiro estágio de inserção nos canais artificiais (p = 0,008). Em relação à força apical, os instrumentos RC alcançaram valores maiores em relação aos instrumentos WO (p = 0,04) no segundo estágio de corte. Conclusão: Os instrumentos Reciproc demonstraram uma eficiência de corte estatisticamente superior em comparação com os instrumentos WaveOne.

Objetivo: O objetivo do presente estudo foi avaliar e descrever as possíveis causas de insucesso endodôntico detectadas nos pacientes atendidos no Projeto de Extensão Tratamento Endodôntico de Molares e Retratamento da Faculdade de Odontologia da Universidade Federal de Minas Gerais, a partir das informações coletadas nas fichas clínicas e radiografias dos casos. Métodos: A revisão de literatura foi feita nos portais da Bireme, Scielo e Pubmed, além de livros textos, dissertações e teses. As palavras-chave utilizadas foram: ápice dentário, falha de tratamento, endodontia, retratamento, odontologia. Os dados foram coletados durante o ano de 2014 e início de 2015, e compreenderam: identificação do dente indicado para retratamento, idade do paciente, se havia ou não sintomatologia, clínica responsável pelo encaminhamento do paciente, tempo de espera até o início do retratamento e radiografia inicial do dente. O universo amostral obtido foi de 82 casos. Foram realizadas análises radiográficas e a classificação do(s) motivo(s) de insucesso do tratamento endodôntico. Resultados: 73 casos (89,02%) apresentaram duas ou mais possíveis causas de insucesso do tratamento endodôntico. As classificações mais presentes neste estudo foram: subobturação (69,5%), presença de lesão periapical (63,40%) e formatação dos canais inadequada (52,2%). Conclusão: Com base nas informações encontradas pôde-se observar que, uma série de fatores pode contribuir, separada ou conjuntamente, para o insucesso do tratamento endodôntico. A desinfecção insuficiente e a obturação inadequada do canal radicular parecem ser as principais causas responsáveis pela maioria dos casos de insucesso, seguida pelos acidentes operatórios e pela ausência de selamento coronário insuficiente.(AU)


Aim: This study sought to evaluate and describe the causes of endodontic therapy's failure to properly treat patients from the UFMG School of Dentistry's Extension Project entitled Endodontic Treatment in Molars and Retreatment The data were collected from patients' files. Methods: The literature review was carried out in the databanks of Bireme, Scielo, and Pubmed, in addition to textbooks, Masters theses, and Ph.D. dissertations. The key words used in the search were: dental apex, treatment failure, endodontics, and dental retreatment. The data were collected during 2014 and the beginning of 2015 and consisted of the following: which tooth was recommended for endodontic retreatment, patient's age, presence or absence of symptomatology, health clinic responsible for referring the patient, waiting time before beginning retreatment, and tooth's initial radiography. The overall sample consisted of 82 cases. The radiographs were analyzed and the cause(s) of the treatment failure was classified. Results: The results showed that 73 cases (89.02%) presented two or more probable causes of endodontic treatment failure. The most common classifications presented in this study were: under-obturation (69.5%), presence of periapical lesion (63.4%), and inadequate root canal shaping (52.2%). Conclusion: Based on the information found, it was possible to observe a series of factors that can, separately or in a group, contribute to the failure of endodontic therapy. Insufficient disinfection and inadequate obturation seem to be the most common responsible causes of failure, followed by operative accidents and the lack of adequate coronal sealing.(AU)

INTRODUCTION: The purpose of this study was to investigate if nickel-titanium instruments with similar designs manufactured by different thermal treatments would exhibit significantly different in vitro behavior. METHODS: Thirty-six instruments each of ProTaper Universal (PTU F1; Dentsply Maillefer, Ballaigues, Switzerland), ProFile Vortex (PV; Dentsply Tulsa Dental Specialties, Tulsa, OK), Vortex Blue (VB, Dentsply Tulsa Dental Specialties), and TYPHOON Infinite Flex NiTi (TYP; Clinician's Choice Dental Products, New Milford, CT) (all size 25/.06) were evaluated. Bending resistance, torsion at failure, and dynamic torsional tests were performed with the instruments (n = 12). Analysis of variance and Tukey post hoc tests were applied. RESULTS: Flexibility was significantly higher for TYP compared with the other 3 groups (P < .0001). With respect to the maximum torque at failure, PV group showed the highest resistance to twisting (torsional strength) among the analyzed instruments followed by VB, TYP, and PTU. The TYP group exhibited greater angular deflection at failure compared with the other groups (P < .0001). The mean dynamic torque scores during simulated canal preparation were highest for TYP (3.01 ± 0.71 Ncm) and lowest for PV (1.62 ± 0.79 Ncm). However, no significant differences were observed comparing groups PTU with TYP and VB and VB with PV (P > .05). The highest mean forces were recorded with PTU (7.02 ± 2.36 N) and the lowest with TYP (1.22 ± 0.40 N). CONCLUSIONS: TYP instruments were significantly more flexible than the other instruments tested. The PV group had the highest torsional strength and TYP, despite being the most flexible, showed similar torsional moments to the other instruments, whereas its angular deflection was the highest among the groups.

Objetivo: Realizar um estudo retrospectivo sobre o índice de fraturas de instrumentos manuais de aço inoxidável e rotatórios de níquel-titânio em tratamentos endodônticos realizados por alunos de um curso de especialização em Endodontia, relacionando a fratura dos instrumentos à fase de instrumentação dos canais. Material e Métodos: Foram analisados prontuários de pacientes atendidos por estudantes dos cursos de pós-graduação em Endodontia do Instituto de Estudos da Saúde (IES) em Belo Horizonte, Brasil, entre maio de 2010 a novembro de 2012. Os prontuários continham um formulário específico para coleta de dados referentes à fratura de instrumentos. As radiografias foram analisadas em negatoscópio sob magnificação por dois examinadores previamente calibrados. Resultados: Dos 501 prontuários analisados, houve 18 casos de fratura de instrumentos. Destes, 10 (55,55%) foram de fraturas de instrumentos rotatórios de NiTi e 8 (44,44%) de instrumentos manuais. Dos 8 instrumentos manuais fraturados, 7 sofreram fratura na fase de exploração inicial. Em apenas um instrumento observou-se fratura na fase de formatação manual dos canais. Conclusão: Para ambos os tipos de instrumento foram verificados baixos índices de fraturas, os quais foram influenciados pela fase de instrumentação dos canais. Os instrumentos rotatórios de NiTi apresentaram maior índice de fratura quando comparados aos instrumentos manuais de aço inoxidável na fase de formatação dos canais.Descritores: Endodontia. Fratura. RESUMO

OBJECTIVE: The purpose of this study was to assess the influence of both the metallurgic and the geometric characteristics of nickel-titanium endodontic instruments on their flexibility. STUDY DESIGN: ProTaper Universal (PTU), K3, and EndoSequence (ES) instruments were evaluated. Their chemical composition and phase constitution were characterized by conventional analytical techniques. Instrument diameter and cross-sectional area were evaluated by image analysis. Flexibility was evaluated in bending tests according to the ISO 3630-1. RESULTS: K3 presented the highest average bending moment (M(B)), followed by PTU and ES. A linear relationship was found between M(B) and the instrument's diameter and cross-sectional area at 3 mm from the tip. Alloy chemical compositions were similar among the files, but transformation temperatures were lower for K3 instruments, which presented only austenite as a constituent phase. CONCLUSION: Nickel-titanium instruments from different manufacturers have unequal behavior related to their flexibility properties. Manufacturing processes changed the phase constitution and transformation temperatures of the instruments investigated and may also have influenced their flexibility.

INTRODUCTION: The purpose of this study was to compare the flexural fatigue and torsional resistance of ProFile GT and GT Series X instruments, taking into account their structural and dimensional characteristics. METHODS: Instrument diameter at each millimeter from the tip and pitch length were the dimensional parameters measured. Chemical composition was determined by energy-dispersive x-ray spectroscopy and phase constitution by x-ray diffraction. Vickers microhardness measurements were performed to assess instrument strength. One group of 20/.06 GT and GTX instruments (n = 10 each) was tested until failure in a flexural fatigue test device, whereas another group of 20/.04 files (n = 10 each) was tested in torsion based on ISO 3630-1. RESULTS: GT and GTX instruments showed approximately the same chemical composition, namely 51at%Ni-49at%Ti (56wt%Ni-44wt%Ti) and contained mainly the beta-phase. GTX instruments showed higher intensity x-ray diffraction peaks and a statistically higher Vickers microhardness. There was a significant decrease in the diameter of GTX in relation to GT instruments from D6 to D9 for 20/.04 instruments and from D4 to D7 for 20/.06 instruments. Pitch length increased along the active part of both instruments, with a steeper increase in GTX. In general, GT Series X instruments were significantly more resistant to flexural fatigue than were similar GT instruments (p < 0.001) but exhibited lower torsional strength (p < 0.001). CONCLUSIONS: Different structural and dimensional characteristics were found in GTX instruments in comparison with GT instruments; this is probably the cause for their higher flexural fatigue resistance and lower torsional strength.

The purpose of this study was to assess how the geometric and dimensional changes made on ProTaper Universal (PTU) instruments in comparison with ProTaper (PT) instruments affected their flexibility and torsional strength. Two groups (n = 12) of each type of shaping (S1 and S2) and finishing (F1, F2, and F3) for both PT and PTU instruments were employed. Tip angle, instrument diameter at each millimeter, and cross-sectional area at 3 mm from the tip were the dimensional parameters measured. Flexibility and torsional strength of each instrument (n = 12) were assessed according to specification ISO 3630-1. Data were analyzed using analysis of variance (alpha = 0.05). Flexibility increased for S1 and F1 PTU instruments and decreased for F2 and F3, whereas torsional resistance increased for S2, F2, and F3. Changes in tip angle were accompanied by variation of tip geometry in all instruments.
